The Vermont State Police says inattention by a commuter bus driver led to a five-vehicle pileup on Interstate 89 in May that injured five people.

The Green Mountain Transit Agency bus was approaching a traffic slowdown for road construction when it rammed multiple vehicles on May 15.

Vermont Department of Motor Vehicle inspectors determined there were no mechanical defects or failures with the bus. Two independent companies reconstructed the accident.

Bus driver Deane Harrington has been ticketed for unreasonable and imprudent speed for conditions.

Following the crash, Green Mountain Transit Agency changed its driver training procedures.
